I live at the messy intersection of medicine, technology, and systems design. By day (and night), I’m an emergency physician in San Francisco. By vocation, I’ve spent two decades building tools like MDCalc and Offcall that millions of clinicians rely on to make better decisions at the bedside. By calling, I push health systems and health tech to confront the gap between what patients need and what our industry actually delivers. I’ve shaped AI strategy at Kaiser Permanente/The Permanente Medical Group, advised startups and policymakers, and co-authored The Physicians’ Charter for Responsible AI. My work spans clinical practice, AI, and healthcare reform—but the thread is simple: doctors and patients deserve better than the system they’ve inherited. I write, speak, and build with a bias toward action. My focus right now: • AI in medicine — building trust, designing guardrails, and ensuring algorithms serve doctors and patients, not the other way around. • System redesign — rethinking physician identity, incentives, and workflows in an era of burnout and overdiagnosis. • Health tech accountability — separating hype from impact, and calling out the contradictions that stall real progress.
