I believe that better transition of care between different healthcare settings, and reduction of hospital readmissions are the first steps towards better health care. As a primary care physician, I work hard to try and keep the patients out of the hospital. Big believer in preventive care, especially in the geriatric population. Spend a lot of time in transitioning patients who have just been discharged from the hospital so that they stay healthy at home and do not get back into the hospital.
