  • I performed biopsychosocial assessments on patients with a range of social issues: mental health, limited prenatal care, lack of maternity resources, lack of health insurance, substance abuse, domestic violence and other family and environmental issues, social program assistance, and for infant loss resources. I updated a watch list for families involved with the Department of Family and Protective Services, and coordinated any pediatric and ante partum home health care. Coordinated discharges with families and the state. I completed documentation daily. I sent clinicals to the insurance companies regularly as well, assisted with any transfers to other facilities. Provided emotional support to families in crisis. Previously worked full time for five years here before shifting to part-time until the hospital closed down.

  • I interview, assess, and complete studies of the homes and families that are pursuing private adoption to be submitted to the court. I also complete home studies for contested custody cases in which the court asks for my professional opinion based on my assessment what the child custody arrangement should be for the best interest of the child. This is PRN, as it occurs when there is a court order for a study.

  • I worked with clients who were currently in the San Antonio State Hospital and prepared those for discharge into community and state supported assisted living homes. Utilizing Cognitive Adaptive therapy, I worked with consumers diagnosed with schizophrenia, schizoaffective disorder, and bipolar disorder to assist them in acclimating to their new home environments and to enable them to complete daily independent living skills. I worked with a team of healthcare providers and insurance company case managers to identify and provide our consumers with any resources and programs they qualify for. I completed environmental functioning assessments to determine any other immediate needs that the clients have. I visited with the clients each week to provide them continued cognitive adaptive therapy services.

  • Through case management I developed both safety plans and Family Plans of Services with clients to work with them toward gaining skills necessary to provide for their children and appropriately parent, while alleviating the reasons they came into foster care. I worked with families in hopes of reunification and made sure their needs were met through employment and government assistance and other social service programs and resources. I referred clients to family, individual, substance abuse, trauma-focused, and crisis counseling. I formulated treatment plans based on the recommendations of the service providers working with the families. I often referred clients to substance abuse evaluations, classes, and treatment centers based on their individual needs. I referred families to domestic violence, anger management, and parenting group classes as well. I conducted home visits monthly to assess the appropriateness and safety of the home environment. I performed site visits to shelters and residential treatment centers to evaluate the safety and compliance of the placements to ensure the foster care licensing standards were met. I monitored the academic progress of foster children, i.e., establishing a working relationship with the schools to follow the academic recommendations for each student by means of progress reports and meeting with their teachers for conferences. I participated in ARD meetings to ensure the child’s individual needs were met per the Individuals with Disabilities Education Act (IDEA).
