LLMs hallucinations aren't random. They have geometric structure. I spent eighteen months studying how language models drift from source material. What I found: when a model hallucinates, its response stays angularly close to the question without traveling toward the evidence. I call this "semantic laziness" - and it's detectable without using another LLM. The Semantic Grounding Index measures this drift. Across five embedding architectures, the signal holds with r=0.85 correlation. It's not an artifact of any particular model. It's a property of the text itself. I built CERT to make this measurable for production systems. Because "run it through another LLM" isn't going to satisfy Article 15 of the EU AI Act. And because the current approach - using AI to evaluate AI - is fundamentally broken. Before CERT: 25 years moving technology from vision to operations. Led 15,000-person workforce data based transformation. Built ML systems from concept to production for venture-backed companies. Published research on AI safety before regulation required it.
